UCSA MEMBERSHIP DUES PAYMENT AND INVOICE ( please print this out for membership payment!!!)

Please print out the USCSA Membership and Dues application. This form has to be mailed in to the National office with payment.

The 2010 USCSA membership dues deadline is November 24th, 2009!! The following late fees will apply – (Late fees do not apply for new institutions)
